Request for the Issue of a Continuous Synopsis Record Form

To: Owners, Managers, Operators, Masters and ISPS Related Personnel of Cayman Islands Registered Vessels

Regulation 3 of Chapter XI-1 of the SOLAS Convention requires that each vessel to which this chapter applies carries a Continuous Synopsis Record (CSR) from 1 July 2004. CSRs will be issued by each Administration to their individual vessel.

Vessels registered in the Cayman Islands will have the CSR issued by the Cayman Islands Shipping Registry (CISR) in George Town. An application form for the issue of a CSR is included in this notice. Application forms can also be downloaded from www.caymarad.org.

An administration fee of CI$188.60 should accompany the application for the first CSR issued to each Cayman Islands registered vessel.

The CSR consists of three forms. “Form 1” contains the required information as issued by the CISR. “Form 2” is used by the company / master to record changes to this information. When information contained in the CSR changes, the company shall record the new information on “Form 2” and record that the information has changed on “Form 3”. Copies of Forms 2 and 3 are to be forwarded to the CISR who will then issue a new CSR with the updated information. The originals of “Form 2” and “Form 3” must be retained onboard for inspection.

An administration fee of CI$94.30 should accompany all applications for the issue of a revised CSR for each Cayman Islands registered vessel
