Seaman's Discharge Book (SDB) Application

The required items to be submitted with Seaman's Discharge Book (SDB) application
are:

• SDB application page 1 and 2 (page one must be original and cannot be a photocopy)
• Please state present rank and capacity being requested
• Copy of STCW endorsement/certificate, Yachtmaster certificate or other certificate
• Three passport sized photos (With applicant’s name on reverse side)
• Copy of current Medical fitness Certificate (within last 2 years)
• Three Specimen Signatures (on page one)
• Record of Sea Service (Sea testimonials, Copied from SDB or Official Document
  – Not a CV)
• Copy of Passport – consisting of photograph, and particulars of holder (include
  signature page).
• Basic STCW Safety Training Certificates and other certificates if applicable
  (Crew other than officer only e.g. Chef, Stewardess etc.)

FYI

Please note that Basic STCW Safety Training is required for all SDB applications; Basic STCW Training Certificates are as follows;

1. PERSONAL SURVIVAL
2. FIRE FIGHTING AND FIRE-PREVENTION
3. ELEMENTARY FIRST AID
4. PERSONAL SAFETY AND SOCIAL RESPONSIBILITY
