Endorsement Application

The required items to be submitted with Endorsement Applications are;

• Application pages 1 through 3 (pages must be original and cannot be a photocopy)
• Two Specimen Signatures (on page one)
• Certified copy a national Certificate of Competency followed by if not included in underlying certificate STCW Endorsement
• Current Medical fitness Certificate (within last 2 years)
• Three passport sized photos (With applicant’s name on reverse side)
• Record of Sea Service within last 2 years (Copied from SDB or Official document – Not a CV)
• Certified copy of Passport – consisting of photograph and particulars of holder (include signature page)
• Certified copy of GMDSS Certificate and Endorsement GMDSS Certificate and Endorsement for Masters and Deck Officers
• Other certificates that are applicable, i.e. Tanker training and endorsement, Passenger Ship training, etc..

FYI

Please note that Basic STCW Safety Training is required for all SDB applications; Basic STCW Training Certificates are as follows;

1. PERSONAL SURVIVAL
2. FIRE FIGHTING AND FIRE-PREVENTION
3. ELEMENTARY FIRST AID
4. PERSONAL SAFETY AND SOCIAL RESPONSIBILITY
